Directions for booking an appointment:
- Choose a location: home, office (Farmington or Hamden) or tele-health. Home visits will incur extra fees. Please read carefully when booking.
- Choose your coverage type**.
- Choose appointment type: Prenatal, Initial or Follow-up. If you are booking with a new baby after a prenatal, it needs to be an "Initial" consult.
